To perform the brow henna procedure safely, please answer the following health questions truthfully. We will keep all disclosed information in a confidential manner and use it only to determine whether you are an appropriate candidate for this procedure.

Consent and Acknowledgments

I agree to have brow henna and eyebrow colouring performed on me by Aliya's Beauty.

Section 1: Acknowledgments and Agreements

Section 3: Use of Likeness and Release

By participating as a client, I permit, authorize, and license the technician(s) and the business and their employees, during this session and for future use, to photograph and video my face and treated area(s), whether obscured or not, before, during, and after the procedure, for purposes such as, without limitation: marketing, promotions, educational materials (including social media, email, portfolio, website, printed materials, advertisements, and digital channels), client records, and demonstrations. I waive any rights of inspection or approval of the finished products, disclaim any right to royalties or other compensation arising out of or related to the use of my likeness, and release the technician(s), business, employees, and legal representatives from any liability related to this use.

The copyright in all such images (including any reproductions) is the exclusive property of the technician and business conducting the service.

I understand that no personal information other than my image will be shared without my written consent and that all photos/videos will be used in a professional manner.

I have read and fully understand the above release. I agree that this consent supersedes any previous verbal or written disclosures. I certify that I have had the opportunity to discuss, and/or have had all questions answered about the above photographic release and that I hereby give express authorization for the acts that have been outlined in this section.

Section 4: General Release and Waiver

I recognize and acknowledge that there are certain risks of personal injury or property damage related to my participation as a client, and I voluntarily agree to fully assume all of these risks, regardless of severity, that I may sustain as a result of participation in any and all activities connected with or associated with the application by the technician of brow henna and/or coloring to my eyebrows using the brow henna technique, including, but not limited to, injuries, damages, and losses arising out of negligent supervision, tort, contract, products, or any other theory of recovery.

I hereby, for myself and for my heirs, assigns, personal representatives, and executors, expressly waive and release any and all claims, now known or hereafter known, against the business and the employees, officers, owners, and agents of the business or technician, as well as their successors and assigns, on account of injury or property damage arising out of or attributable to my participation as a client, whether arising out of the negligence of any releasees or otherwise, to the fullest extent permitted by law. This release is intended to be as broad and inclusive as permitted by the laws of the jurisdiction in which the business is located. I further agree to defend, indemnify, and hold harmless the releasees against any losses, claims, damages, liabilities, or costs, including attorneys' fees, resulting from any claim brought against the releasees arising out of or relating to this waiver and release. Should any part of this waiver and release be deemed unenforceable, that part shall be deemed severable and the remainder shall continue in full force and effect.

I understand that this waiver and release are intended to be as broad and inclusive as permitted by applicable law, and I agree that if any portion is held invalid, the remainder will continue in full legal force and effect. I have read this waiver and release and voluntarily sign it. I further agree that no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made.

I have read the information in this waiver thoroughly. I understand that I have given up substantial rights by signing it. I have the capacity to provide informed consent and I am signing this waiver and release freely and voluntarily.
